Default DVRs

Are DVRs a seperate box with your cable system or are they part of it. I have been considering switching to digital cable this winter ( I downgrade to basic every summer). I was wondering if you have to buy a box on your own or pay an additional monthly fee for the service. Can you really rewind the shows? I don't remember the last time I watched an entire episode of a favorite show on tv. Probably before I had children. Also if you don't have to buy your own DVR box is it cheaper in the long run to purchase or is this one of those things you want to wait for a few years until the price goes down? How easy are they to learn to use? But most importantly, as long as I have a working VCR is it worth it to get a DVR or just something that starts out exciting then drops down the ladder after a while?
